4) Payment & Insurance

• We accept most major insurance plans; eligibility must be verified.
• Co‑pays/deductibles are due at time of service unless otherwise arranged.
• Financial assistance may be available; payment plans can be arranged.

5) Privacy & HIPAA

We safeguard your information in accordance with HIPAA and our Privacy Policy. PHI is used/disclosed only as permitted by law, for treatment, payment, and healthcare operations, or with your written authorization.

6) Website Use

• You agree not to misuse forms, attempt unauthorized access, or disrupt services.
• Website content is for informational purposes and not a substitute for medical advice.

7) Termination

We may discontinue website access for misuse, and clinical services may be ended consistent with program rules, safety, and ethical obligations.

8) Disclaimers & Liability

Website materials are provided “as is” without warranties. To the fullest extent permitted by law, ABL disclaims liability for indirect or consequential damages arising from site use. This does not limit any patient rights under healthcare law.
